journals.lww.comSynchrony between the patient and the ventilator is defined as the appropriate interaction between the two, where the ventilator recognizes patient’s effort and provides support for breathing at the right time—that is, inspiratory and expiratory times matches with patients’ times (patient’s neural time equals to ventilator times). In our humble opinion, the mismatch between the patient and the ventilator occurs because we set the ventilator to what we believe is best and not really to what the patient needs (we depersonalize mechanical ventilation). This often leads to a strategy of deep sedation and neuromuscular blockade; it is easier to ventilate the patient according to our wishes, although it is not an adequate strategy to improve the prognosis of most of our patients.
