The occurrence of ICU-CA in critically ill patients with advanced age (≥ 90 years) is relatively rare. The observed mortality in the ICU and hospital was exceedingly high. Notably, providing cardiopulmonary resuscitation (CPR) did not result in any patient being discharged alive from the hospital.

For very elderly patients, where ICU admission is deemed justified following thorough evaluation by experienced medical staff, supportive care should be prioritised.

However, our findings suggest that administering CPR in these cases may not be advisable.

Nevertheless, decisions regarding for or against advanced life support must be personalized.
