The prevalence of do-not-intubate (DNI) order was frequent in patients with acute respiratory failure (ARF) admitted to the ICU, being related to age, comorbidity, frailty and the underlying disease.

non-invasive ventilation (NIV) as a respiratory support measure to restore the previous state of health was an effective tool in these patients.

The mortality of patients with DNI order was high, both in-hospital and in the long term, with patients with chronic respiratory disease being those who can benefit the most from NIV.

5,972 patients were analyzed, 1,275 (21.3%) presenting a DNI order.

The mean age was 68.2 ± 14.9; 60.2% were male.
