Despite significantly improving the treatment of blood cancers, CAR T-cell therapy still faces significant hurdles due to its complex and varied toxicities, which require ongoing research to optimize management; while understanding of these complications is growing, the high rate of side effects remains a major clinical concern, although advancements in treatment strategies have thankfully led to a decline in mortality associated with these toxicities.

With the rapid increase in the utilization of CAR T-cell therapy, the number of patients with severe toxicities and ICU admissions is also rising.

Given that these potential adverse events may present with a wide array of symptoms, severity, and timing, it is crucial for intensivists and other providers involved in the care of these patients to understand the presentation, grading, and optimal management of these toxicities.
